What a bunion actually is
A bunion (hallux valgus) is a gradual change at the base of the big toe, where the joint drifts out of alignment and the toe leans towards its neighbours. The bump you see is the joint pushing outward. It tends to develop slowly over years, and genetics and foot mechanics play a bigger role than footwear alone.
Importantly, a bunion is a structural change. That's the key to understanding your options.
What conservative care can - and can't - do
Non-surgical care is genuinely useful and is the right starting point for most people. What it does well is manage symptoms and slow aggravation. What it can't do is reverse the deformity - only surgery does that.
- Footwear changes: wider, softer shoes reduce pressure over the joint
- Orthoses & padding: offloading and correcting contributing mechanics can ease pain
- Activity & strengthening: keeping the foot strong and mobile helps function
- The limit: these control symptoms but won't straighten the toe
When surgery is worth considering
Surgery becomes a genuine option when conservative care is no longer keeping up - not simply because a bunion exists. Correction realigns the bone and soft tissues to restore a straighter, more functional joint.
- Persistent pain: the bunion hurts regularly despite footwear and orthoses

- It limits your life: walking, activity or footwear choice are meaningfully restricted
